Background
The interRAI Palliative Care (interRAI PC) is a comprehensive, standardized instrument for evaluating the needs, strengths, and preferences of those in palliative care or hospice settings. The interRAI PC has been designed to be compatible with other internationally used interRAI instruments for long-term care facilities, home care, community mental health, in-patient mental health, acute care, and post-acute care. The compatibility of assessment elements improves the continuity of care through a seamless health assessment system across multiple settings, and promotes a person-centered approach to care.
Target Population
The PC has been designed for use with an individual approximately 18 years of age and older. The PC is sufficiently comprehensive to be of special value in the care of any person near the end of life. It can also be used for individuals who have significant palliative care needs, such as persons in intensive care units.
